From a macro perspective, we can track population health through our system to tracking diseases within a population. Our system is customizable with multi-tenant, partitioned clients.   We use ICD10/ICD11, INOMED, CIE II to track illness and health improvements 

Chronic Care Management (CCM) focuses on patients with multiple chronic conditions, while Principal Care Management  (PCM) focuses on a single, high-risk chronic condition. Our system tracks them both using international standards and CPT codes within a process workflow.
